Financial Policies and Insurance

As a courtesy to our patients, our office will verify, process and accept insurance benefits when possible. To provide this service, we will ask for specific and confidential personal information such as a street address and social security number. We will work with your insurance company to arrive at the best possible ESTIMATE of your co-payment for your treatment. In order to process your insurance benefits, we ask that all estimated co-payments be paid on the day of treatment while we wait for the balance to be paid by your insurance plan.

If insurance benefits are reduced or denied as determined by your insurance plan, the balance shall be the full responsibility of the patient and/or the guarantor responsible for the account with our office. We expect insurance payments to be received no later than 60 days after a service is rendered however, at times this will require that the plan subscriber or the patient submit additional information to allow a claim to be processed. It is important that you are prompt in returning information requested by your insurance plan for any claim.

For patient convenience, our office accepts cash, personal checks with proper ID, and major credit cards for payments. We can also provide health care financing through a third party company.
