Prior to your visit, we will ask you to provide our office with personal information in order to register you as a patient in our system. Our office adheres to strict privacy practices for personal and protected health information. A complete copy of our privacy practices, protected health information uses and disclosures and your rights regarding these policies can be made available for your review. Our privacy practices are in full compliance with the HIPAA privacy regulations set forth by the Federal government.
medical and health history information
In order to provide the best possible care, we will ask for your complete medical history on arrival to our office. Our staff will be reviewing your health, medication and allergy history with you. It is important that this information be accurate and complete in order to provide you with the safest possible treatment. Occasionally, we will recommend that x-rays be taken for a complete diagnosis and to determine a treatment plan. Our use of digital x-rays and electronic medical records helps to reduce radiation to patients and streamline your appointment.
